Components of Quality Documentation


Case documentation should answer the questions: Who? What? Why? Where? When? and How?

Answering these questions helps others to understand what’s been going on and why certain decisions were  made. Quality documentation includes the following components:

First Person Example

"I spoke with Andrea Smith, Jeffrey’s foster mother, about the transportation I’ve scheduled with the Kennebec Valley Community Action Program (KVCAP) so that Jeffrey can get to his therapy appointment each week. On Thursdays at the end of the school day, KVCAP will pick up Jeffrey at school and  take him to his therapy session. Following the therapy session, KVCAP  will transport him back to the foster home."


Larry question: Should the documenter indicate the elapsed time of the therapy session?

Larry's suggestion for a quiz opportunity: Why not first provide a lousy example of documentation and have the learners identify “what’s wrong with this picture?” As a matter of fact, why not provide 3 or 4 examples of both good and not-so-good examples for the learners to analyze and react to?
