Documenting Accurately to Support
Decision Making



Introduction


In this lesson, you understand the crucial role of documentation in the child welfare case planning process. You  learn that unbiased, accurate, and concise documentation is critical to making decisions about child safety, permanency, and well-being.

Narrative entries document your purposeful and substantive contact (communication?) with persons important to the child’s case.
